The 2023-2024 Transportation Verification Form is available on your Skyward Family Access. Please take a moment to complete this form at your convenience. Any student requiring transportation to and/or from school must complete the Transportation Verification Form. The deadline to ensure transportation on the first day of school is July 24, 2023. Failure to complete the form by this date may result in a delay of transportation services provided.
To find the form, please click Skyward Family Access to access your account:
- Click on the Online Forms tab on the left side of the screen.
- Click Fill Out Form on the 23-24 Transportation Verification Form for each student enrolled
